Required Steps and Training Directions to Become a Licensed Foster Parent

In preparation for the training, please review and complete the following items to receive your foster parent license:

  1. Make an Appointment to get fingerprinted - Foster parents and any other individuals residing in the home (18 or older) will need to get fingerprinted at the State Patrol Office located on 108th & I street.

  2. Complete a Health Form, click this link to print the form.

    • Then, schedule your health appointment with your physician.

    • Fill out and sign Part A of the form.

    • Ask your physician to complete section B.

    • Submit your form back to our team, licensing@apexfamily.org

  3. Complete the Department of Health and Human Services (DHHS) self-assessment:

  4. Follow the directions below to complete the required trainings. There are several trainings that must be completed prior to the first NDTC New Foster Parent class.
