Required Steps and Training Directions to Become a Licensed Foster Parent
In preparation for the training, please review and complete the following items to receive your foster parent license:
Make an Appointment to get fingerprinted - Foster parents and any other individuals residing in the home (18 or older) will need to get fingerprinted at the State Patrol Office located on 108th & I street.
Complete a Health Form, click this link to print the form.
Then, schedule your health appointment with your physician.
Fill out and sign Part A of the form.
Ask your physician to complete section B.
Submit your form back to our team, licensing@apexfamily.org
Complete the Department of Health and Human Services (DHHS) self-assessment:

You will not be allowed to attend the New Foster Parent Training class until we’ve received your certificate.
Follow the directions below to complete the self-led required training. There are several trainings that must be completed prior to class, you won’t be allowed to start class until the trainings are completed.

Complete the Foster Parent Post Test
Select the Home Type, Agency Supported Home
Next to Agency Name, enter Apex Family Care
Enter your first and last name.

Our administrative assistant will receive notification of the completed quiz. They will send the training certificate to our licensing team. No further action is required from you. You must get 80% or more to pass.
