  • You’re required to submit 12 CEU hours per year based on your provider renewal date.

  • Use the form below to submit your training certificates. Submit only one certificate at a time.

  • For trainings without a certificate (for example: online webinars, books, videos, therapy services), you must submit a valid learning summary. Click here to complete the learning summary.

RENEWAL CEUs
You can use the following resources to obtain Renewal CEUs:

  • Apex led trainings and guest speakers - visit our Events Page to view and register for upcoming trainings

  • Other agency trainings (examples: Boys Town, Project Harmony). A certificate of completion must be provided.

  • Online trainings or webinar. A certificate of completion or valid learning summary must be provided.

  • Books, videos, therapy services, etc. A valid learning summary must be provided.

HIGH LEVEL CEUs
Upon placement, we’ll contact you with a curriculum to complete your trainings.
